At Kids Docs Pediatrics, we go beyond checkups by offering free value-added services and rewards to support your child’s health journey. Whether it’s completing a well-child visit, staying active in extracurriculars, or managing chronic conditions like asthma or diabetes, our goal is to encourage preventive care and reward healthy habits
All rewards listed below are available once per child per benefit year (September 1, 2024 through August 31, 2025). Each benefit has specific age requirements and qualifying activities.
$50 Gift Card for Extracurricular Activities
Get rewarded when your child participates in any extracurricular activity like sports, music, dance, art, or school clubs. A $50 gift card is available once per year for ages 2–18 after completion and submission of details.
Eligibility: Ages 2–18
Requirement: Must complete an approved extracurricular activity
Claim: Submit the activity name with child and parent details
$25 Gift Card for School Supplies
Complete a well-child visit for your child between September 1, 2024, and August 31, 2025 to receive a $25 gift card to help cover back-to-school essentials. Available once per year for children aged 3–17.
Eligibility: Ages 3–17
Requirement: Complete one well-child checkup within the benefit year
Claim: Provide visit date and basic patient information
$20 Gift Card for Annual Dental Cleaning
Support your child’s dental health by scheduling a routine cleaning, and receive a $20 gift card once per year. This benefit is available for children ages 2–20 and requires documentation of the visit.
Eligibility: Ages 2-20
Requirement: One dental cleaning with any dental provider
Claim: Submit the cleaning date and patient information
Free Asthma Toolkit
Children with an asthma diagnosis can receive a complimentary asthma care kit, including educational tools, daily management items, and parent-friendly resources to support long-term health.
Eligibility: Children with a diagnosed asthma condition

Free Diabetes Toolkit
Eligible children diagnosed with Type 1 or Type 2 diabetes can receive a free diabetes management toolkit with care guides, monitoring tools, and tracking sheets to support home-based care.
Eligibility: Children diagnosed with Type 1 or Type 2 diabetes
Requirement: Daily care resources, educational materials, and support tools
Claim: Submit confirmation of diagnosis and member information
$25 Gift Card for Well-Child Visit
When your child completes a routine well-child exam within the benefit period, you can claim a $25 gift card as a reward for prioritizing preventive care. This applies once per year for ages 3–20.
Eligibility: Ages 3–20
Requirement: Routine well-child checkup between Sept 1, 2024 – Aug 31, 2025
Claim: Submit visit date and all required patient/parent information

Key Guidelines for All Rewards
- Each reward is available once per child per year
- Eligible activities must be completed between September 1, 2024 and August 31, 2025
- Your child must be eligible at the time of reward processing
- Documentation (e.g., visit date, diagnosis, activity proof) may be required
- Rewards will be mailed or distributed after verification
- Supplies and toolkits are limited and offered while available
